Provides services that are ordered by the physician, included in the Plan of Care, permitted to be performed under state law, and is consistent with the home health aide training. The Home Health Aide understands and adheres to established Agency policies and procedures, including infection control and prevention. Provides hands-on personal care as assigned. Performs household services essential to health care in the home as assigned. At Advantis our home health aides assists with ambulation and exercises in order to keep patients active and well. Maintains open communication with the patient, representative (if any). caregivers and family. The goal of the home health aide is to keep patient comfortable and make sure that their personal needs are met while they are under our care.
Patients can receive

Reports changes in the patient’s condition and needs to the RN or other appropriate skilled pro tessional
